FREE MLTF STUDENT ACCESS (The Angus Plan)

This plan will provide filtered dial-up Internet access for use with a student’s MLTI Apple iBook. This access is intended for educational purposes and not as a substitute for an existing family Internet service plans. The Foundation will pay for this access at no cost to the student or the family. Some service limitations may apply.
Eligibility: In order to qualify for the Free Access plan, a student must meet the guidelines of the Free and Reduced School Lunch Program (FRSLP) for the 2007-08 school year and must not currently have access to the Internet from home. The school must allow the student to take his/her iBook home outside of school and have an Acceptable Use Policy that includes the use of the iBook at home.
Process: Each interested student must complete and return the Home Access Sign-up and Usage Agreement form and the FRSLP Sharing form. These forms require a signature from the student’s parent or legal representative. The student should return these forms to a designated person within the school. The school should fax completed forms to:

Upon receipt of the completed forms, the Foundation will create an account for the student and send the students login information to the designated person at the school to allow for the setup of the students iBook.

DISCOUNTED MLTF ACCESS (GWI Student/Teacher Plans)

In partnership with MLTF, GWI will also provide discounted, home-based dial-up and high-speed broadband Internet access to eligible Maine public school students and teachers. Qualifying students and teachers are those identified by the MLTI (laptop program) and issued an Apple iBook computer with a registered asset tag or other unique inventory identification issued by the school The discounted program subscriptions will be billed directly to the parents of students and/or teachers as authorized by same. The intent of the program is to offer discounted, filtered Internet service for educational purposes, content managed under the auspices of the MLTF. (Note: filtered content will initially only be available on the Dial-up accounts, as technical preparations are made to offer the filtering rating on other broadband discounted services in the coming months). Discount program rates expire upon termination of individual’s eligibility under the laptop program, discontinuance of the laptop program itself, and/or expiration of the MLTF program with GWI. Customers will automatically be billed the prevailing standard retail service rate(s) within 30-days of termination of eligibility.
In an addition to the discounted Internet plans above, and at our standard bundled rate, GWI can provide its telephone service to many area homes. Named GWiLine, you can include our new telephone service option with your discounted MLTF Internet subscription. GWiLine allows you to optimize your account plan for both services, and you can keep your existing telephone number. (Must be in a qualified Verizon service line area, other limitations may apply). For most homes with a working telephone line, no new lines or additional equipment are necessary. One simple call to GWI, and you can be hooked up to a future of unlimited US and Canada calling at one flat monthly rate.

ACCOUNT RENEWAL

GWI will require annual account renewal authorization for Discounted MLTF Access Plans. A web-based authorization request-for-renewal form will be available on this web-site for eligible students and teachers to reactivate accounts using newly assigned iBook asset tags numbers for verification, and at the beginning of each new public school year. MTLF customers may enroll as new customers or cancel access plans at anytime. No temporary or seasonal disconnects allowed during summer breaks, school vacations, or for other reasons.
